I am under 1+3 HSMP visa(initially granted in June 2004 and extension in 2005) and now my next extension is due this June. I have not got any new HSMP approval letter but I want to apply for extension under the new form. I am confused that should I consider myself initial entrant to tier-1 and fill section 3a of the form or shall I fill section 3c for extension only. I know in both case I have to pay £750 but not sure about the right section. All comments welcome.

You are making an extension so you should complete section 3C.
